If your plan requires a Referral to be seen by a Specialist, please obtain it from your Primary Doctor prior to your visit.
Our online scheduler allows patients to self-schedule regular appointments (NOT SURGICAL appointments, which can only be scheduled by the office). If you are an established patient, you should see a "welcome back!" notice appear after validating your name and date of birth. If you do not see this even though you have been a patient of the practice, please call the office after scheduling your appointment to verify your information, before filling out the registration form which will be e-mailed to you.
After scheduling an appointment, you will receive a registration link from eDerm. Login with the patient's last name and date of birth. If you didn't receive the email, please check your Spam Folder. If you still can't find the email or can't login, please call the office.
If you would like your appointment to be Telehealth rather than in-person, please include 1-3 quality photos of the condition that you would like to address (you may put these photos in the "Other Registration Documents" section of the registration form).
